Support pads do not bunch up, so order the smaller size for dogs in between sizes. Still not sure? Try the Towel Test: Fold a bath towel to 5.5 inches wide to simulate the Large/X-Large, then sling it under your dog's belly. If that's too big, then re-fold the towel to 3.5 inches wide to simulate the Medium size
